- A contribution may be made directly to the United Way by completing and forwarding this form to the United Way or you may choose payroll deduction by printing the form, completing and signing and forwarding a hard copy to our payroll department.
- Please be sure to specify the designation of your gift to one or more of your charitable interests sponsored by the United Way.
